The Five Wishes Workshop

Where do I want to spend the last days of my life? Under what conditions do I want to be kept alive and when do I want to be allowed to die?

At CBB, we are participating in a community-wide effort called “The Alliance for Living and Dying Well.”  The Alliance is working with us, with local churches, retirement communities, and with Visiting Nurse and Hospice Care and the SB Hospice, to encourage everyone to begin holding these conversations, using the “Five Wishes,” an Advance Health Care Directive used by Cottage Hospital and other hospitals around the country.  (Click here for a recent Noozhawk article about the Alliance).

Come to this workshop and learn about The Five Wishes document  and to discuss your own wishes with a trained facilitator (bring along a member of your family too).
